Send completed packet, insurance card, and diagnostic report for us to review.

VERIFICATION

Await confirmation of openings as well as insurance verification and authorization.

ASSESSMENT

Once authorized, discuss next steps and set up a date and time for an assessment.

BEGIN SERVICES

A new authorization will be obtained for services to begin so the patient may proceed.

Do you take older children?

We do take teenage children if there are other children they can socialize with at the clinic. We currently do not have a social group.

There is a wait list for the afternoon hours, but there are openings for the daytime.

Do you provide transportation?

Transportation is only provided for Milford, Connecticut students coming after school and is arranged and approved by the child’s school.
